On Thursday September 26, 2024, the Marion County Public Health Department in partnership with Market Decisions Research sent out the first round of invitations to complete the Marion County Community Health Assessment Survey. The letter should include a QR code that will send you directly to the survey online. Options are available to take the survey via phone or paper, or in languages other than English. The envelopes also have $1 in them. Once you complete the survey, you will be able to claim a $14 gift card from a list of vendors.
