Only a parent or legal guardian will receive a child’s immunization record.

If you or your child received immunizations in Indiana, we may be able to provide you with an Immunization record. Mandates requiring providers to submit immunization records to the Children and Hoosier Immunization Registry Program (CHIRP) have only been in place for a few years. While some providers and schools have always shared these records, there were many in the past that chose not to. If we do not have your record, check with your physician, any previous physicians or your last school.
